This form is used when you are having issues getting Coventry to honor a 1/1/09 effective date. You must have proof of delivery (like a fax confirmation form) to go along with this form.
The actual form is in our Resource Center. Put in Coventry as the Company and “Forms” as the Resource.

I have not been paid for two months from Coventry. and have over 50 apps in the pipeline. I have several people that I wrote that don’t even show up on the application tracker on their website.They still owe me on apps that have 12-1-08 effective dates. Don’t feel alone. They told me I would be paid soon whatever that means.
I haver had the worst experience with coventry…I would not for them again

You are correct, but allow me to expand on your comment a bit.
The “MAPD to MA-only with PDP” Open Enrollment change would be to a Private Fee for Service MA-only and stand alone PDP. The PPO and HMO MA only plans would not allow a stand alone PDP.
Thus, if switching a current MAPD member, the choices are:
1. Another MAPD (either HMO, PPO, Special Needs or PFFS with integrated drug plan)
2. MA-only PFFS WITH stand alone PDP
3. Original Medicare with stand alone PDP.
I’m sure you know this, but I wanted to clarify for others reading this blog!

They stopped paying me comp becacuse they said my licensed was expired! It of course isnt, I told them that and they asked me to email them a copy of my license. I did, but its the same exact one I sent up with my contracting.

Of course Coventry did not pay any commissions on Friday, instead they tell me they will be paying them on Saturday. They cut the checks at Tecport Harrisburg. I really need to start getting paid for my apps. I am thinking seriously of going to Harrisburg and try to get to the bottom of this issue…. Any ideas of what I could and/or should do to start getting the commission ball rolling??? If anyone else is not getting paid, meet me in Harrisburg!!!
There needs to be a form to send to the idiots at Coventry that tells them that people can change from an MA-PD to a MA and a PDP during open enrollment. This has been an issue every year and you would think that they would have it right by now!
