Universal American updates Guidance on Scope of Appointment Form

Universal American states that their guidance, based on CMS guidance, remains that for in-home appointments, every effort shoud be made to complete the Scope of Appointment form in advance. However, if this is not feasible, the Scope of Appointment form may be signed immediately prior to the start of the in-home appointment.
To access the Scope of Appointment form, visit our Resource Center and select American Progressive (Universal American) as the Company and CMS Approved Materials as the Resource.
