I’ve heard from one of the companies now that this form is supposed to be used for ANY AND ALL Medicare Advantage companies as an “interchangable” Scope of Appointment or Confirmation of Appointment form.
This seems to make much more sense than having a seperate form for every company when you don’t really know which company might be best suited for the client.
You will find this form in my “Resource Center“. To find it, simply put in “CMS Approved Marketing Materials” as your “Resource” (leave everything else All) and click submit!
For the time being, I’m going to leave all of the other company specific Scope of Appointment forms on the website until and get a few more verifications that this is the appropriate form to use in all instances, however, my source is pretty good!

Agreed, there is alot that’s silly about that form. Like another poster said, we can’t talk about anything that is not a health product on these appointments so the form is almost useless. It only serves to make us look less professional and to take care of the H… agents who enrolled people in $0 premium MAPD plans when they only wanted to get a Part D plan, and OOPS! they forgot to explain anything such as benefits, co-pays and OOPMs (that whole Summary of Benefits thing) that we’re required to review fully.
What’s interesting about this letter is that it can be used for any plan, but each plan has to file their form with CMS. It would seem odd to mail them the Company X form but talk about company Z.
I’m NOT advocating that an agent should send them a letter for each company. I just think it is silly to have to submit to CMS if the company is using the CMS model letter.

YAY, something that makes sense comes out finally… at the last minute… before the beginning of the AEP today… Now if only the darn form really fixed a valid beneficiary concern and had a good purpose besides just increasing the amount of postage spent here locally. Now where are those darn comp schedules I should also have by today? Oh yeah, that’s right I got one from the company that caused the above form to be designed… Gotta go fight the good fight now and take care of my customers. HAPPY TRAILS!!!
