2011 Update: I’ve updated some information on the 2011 Generic Scope of Appointment Form here!
Here is a Generic Form. I asked one of my companies to check with CMS to see if this can be used and/or if it matters whether or not you use the company form which is specific to the product you are selling. I’m awaiting feedback on this point.generic_sales_appointment_confirmation_form
