There has been a form change regarding the Montana Medicare Supplement Application for Royal Neighbors of America. The new application contains revised medical questions and a change of logo.
The NEW form number is “3500-MT; Rev 7-2007″.
Please replace ALL other Montana Medicare Supplement Applications in your possession with the new application included in this packet immediately.
DO NOT use the OLD application effective July 7, 2008.
The old application will be accepted with a signed date PRIOR TO July 7, 2008. Any old application signed July 7, 2008 and AFTER, will require the new application for the business to be accepted. Applications submitted on the old application form with a signed date of July 7, 2008 and after will not be processed and the new application will be requested.
A new Reinstatement Application form (Form 3550; Rev. 7-2007) is also available for use in conjunction with the revised 7-2007 Application. This will NOT replace the existing form as the existing form can be used by certificate holders underwritten on the previous application.
